    Nature of Requirements: The following is a summary of the statement of work for this requirement.

    The CSC wishes to obtain physiotherapy services to provide essential physiotherapy treatments and services to offenders referred by Health Services professionals according to the standards of the Ordre professionnel de la physiothérapie du Québec. The physiotherapy services are required at the following Correctional institutions: Donnacona, Port-Cartier and Federal Training Center (site 600 and site 6099).

    Objectives: The Contractor must provide clinical physiotherapy care to federal offenders and collaborate with the institution’s multidisciplinary health services team, which includes, but is not limited to, general practitioners, nurses, psychologists, social workers, occupational therapists and other related health care professionals.

    Assess offenders, diagnose physical condition, develop a treatment plan by providing written consultation notes on the CSC assessment form.

    Deliverables:

    Produce long-term rehabilitation plans and provide treatments according to the plans;

    After every treatment, complete the evaluative report on the relevant CSC form.

    Term of Contract:

    Period of the Contract: The Work is to be performed during the period of the contract is from date of contract award to August 31st, 2020 with the option to renew for two (2) additional one-year periods.
