    This requirement is for the department of Fisheries and Oceans Canada.
    
    Title:    Health Care services at the Canadian Coast Guard College
    
    Introduction: 
     
    The Canadian Coast Guard College in Sydney, N. S. requires the services of a health care specialist (registered nurse practitioner or physician) to provide the following services in French and English:
    
    	Examine Officer Cadets who report sick for the day. Their illness to be evaluated and follow up with doctor referral, if required. A health record for every visit will be established. Provide emergency care in occupational and non-occupational injuries and illnesses in accordance with standing medical directive during the hours the health care specialist is present at the College.
    
    	Conduct adequate follow up and rehabilitation of the ill and injured. This includes referrals to physicians, community agencies and institutions and College resources.
    
    	Work collaboratively with an off-site family physician to provide a range of primary care services.
    
    	Create electronic records, backed up weekly on CD Rom, and printed and filed within a locked cabinet located in the health assessment office.
    
    	Ensure that information of a non-confidential nature, required for training purposes, is communicated with the office of the Superintendent of Officer cadets on a daily basis.
    
    	Conduct health assessments, both on individual and community basis, develop and provide health education and counseling to individuals or groups as required,
    
    	Determine population need and develop, implement and evaluate population health promotion and strengthening primary health care programs.
    
    Estimated Value: 
    The total value of any contract(s) emanating from this RFP shall not exceed $51, 840.00 per year, including all applicable taxes.  (12 hours per week, 48 weeks per year)
    
    
     OPTION TO EXTEND THE CONTRACT:
    
    The Contractor grants to Canada the irrevocable option to extend the term of the Contract by up to two (2) additional one (1) year periods under the same conditions.  The Contractor agrees that, during the extended periods of the Contract, it will be paid in accordance with the applicable provisions as set out in the Basis of Payment.
    
    Canada may exercise this option at any time by sending a written notice to the Contractor at least 15 calendar days before the Contract expiry date.  The option may only be exercised by the Contracting Authority, and will be evidenced for administrative purposes only, through a contract amendment.

    2) Reliability Status for the resource(s) (issued by the Canadian and Industrial Security Directorate at PWGSC)
