Optional Work: You have to submit a separate proposal for supply and install 10 cell doors, type A with service hatches (K121 to K130). For service is required for: Correctional Service of Canada Archambault Establishment Ste-Anne-des-Plaines, Quebec IMPORTANT **MANDATORY VISIT** It is mandatory that the Bidder or a representative of the Bidder visit all of the work site. Arrangements have been made for sites visit to be held on: Friday June 26, 2015 at 09:00 AM. Correctional Service of Canada Corcan Archambault Establishment 242 Montée Gagnon Ste-Anne-des-Plaines, Quebec J0N 1H0 ** Special instruction ** Before being admitted to the facility/site, the Contractor's personnel must undergo a local verification of identity or their information by Correctional Service of Canada (CSC). CSC reserves the right to prohibit access to the facility/site at any time to the contractor's personnel. Contractor's who wish to attend this mandatory visit must complete the ''Access to an institution'' (SCC 1279) refer to Annex «D», sign it and send it AT LEAST FIVE (5) WORKING DAYS prior to the visit, to the following email address: patrick.raymond@csc-scc.gc.ca. It is important that the Bidder's to put back the original copy to Mr. Patrick Raymond during the visit. Bidders are requested to communicate with the Contracting Authority at least FIVE (5) day(s) before the scheduled visit to confirm attendance and provide the name(s) of the person(s) who will attend. Mr. Émile Guernon at (514) 496-3585 prior to the visit to confirm attendance. Bidders will be required to sign an attendance form. Bidders should confirm in their bids that they have attended to all site visit. Bidders who do not attend or send a representative will not be given an alternative appointment and their bids will be rejected as non-compliant. Any clarifications or changes to the bid solicitation resulting from the site visit will be included as an amendment to the bid solicitation. The supplier will be selected following an evaluation based on mandatory technical and financial criteria specified in the Statement of requirements that form part of this Request for Proposal. This Request for Proposal include security requirements for CSC only. As per the Integrity Provisions under section 01 of Standard Instructions 2003, offerors must provide a list of all owners and/or Directors and other associated information as required. Refer to section 4.21 of the Supply Manual for additional information on the Integrity Provisions. The requirement is subject to the provisions of the World Trade Organization Agreement on Government Procurement (WTO-AGP), the North American Free Trade Agreement (NAFTA), and the Agreement on Internal Trade (AIT). All inquiries and other communications concerning this Request for Proposal must be sent in writing only to the contracting authority named in the Request for Proposal. Public Works and Government Services Canada provides services in Canada's two official languages.
