B- TO EXTEND 2nd PERIOD CLOSING DATE (ANNEX B) FROM JULY 13th 2015 2:00PM TO JULY 15th 2015 2:00PM. C- TO MODIFY ANNEX B CALENDAR CLOSING DATE (SEE NEW DOCUMENT ATTACHED CALLED MODIF 002 - WORK DOCUMENT). ************************************************* DESCRIPTION: This Regional Individual Standing Offer (RISO) request involves the provision, on an as and when ordered basis, of Cheese. IDENTIFIED USER: Veterans Affairs Canada, Ste.Anne's Hospital, Ste-Anne-de-Bellevue, province of Quebec. PERIOD OF STANDING PROPOSAL: From February 1, 2015 to January 31, 2016, divided into two (2) six- (6-) month periods. Note: Offers will have to be submitted for each period, as per the schedule in Annex "B". One or more Standing Offers could be issued for each period. DELIVERY: DELIVERY VAC Delivery address Delivery hours and details Veterans Affairs Canada Ste. Anne's Hospital Magasin des vivres 305, boulevard des Anciens Combattants Ste-Anne-de-Bellevue (Quebec) H9X 1Y9 Mondays (AM) or as needed. Delay: Delivery will be made in accordance with the delivery date mentioned on the call-up document. QUANTITIES: The estimated quantities for each of the required items can be found in Annex B - Working Document - List of products. APPROXIMATE SCOPE OF THE STANDING PROPOSAL: The approximate scope of the Standing Proposal is $65,000.00 for the period from February 1, 2015 to January 31, 2016 inclusive. INTRODUCTION OF A NEW CODE OF CONDUCT CERTIFICATION: Pursuant to section 01 of Standard Instructions 2006, offerers should provide, with their offer or promptly thereafter, a complete list of names of all individuals who are currently directors of the Offerer. Canada may, at any time, request that an Offerer provide properly completed and Signed Consent Forms (Consent to a Criminal Record Verification form - PWGSC-TPSGC 229) for any or all individuals named in the aforementioned list within a specified delay. TRADE AGREEMENTS: "The requirement is subject to the provisions of the North American Free Trade Agreement (NAFTA) and the Agreement on Internal Trade (AIT)." Delivery Date: Above-mentioned The Crown retains the right to negotiate with suppliers on any procurement. Documents may be submitted in either official language of Canada.
