    Pursuant to GI01 of General Instructions to Bidders R2710T,
    bidders must submit the following as part of their bid: 
     			
    a.	a complete list of names of all individuals who are currently
    directors of the Bidder and
    					
    b. 	a properly completed and signed Consent to a Criminal Record
    Verification form

    	individual named in the aforementioned list. 
    
    The above should be completed and submitted with the bid by the
    bid solicitation closing date.  If the above is not completed
    and submitted with the bid by the bid solicitation closing date,
    the Contracting Authority will so inform the Bidder and provide
    the Bidder with a time frame within which to meet the
    requirement. Failure to comply with the request of the
    Contracting Authority and meet the requirement within that time
    period will render the bid non-responsive.
    
    Work Description: 
    Work of this contract comprises bioremediation of petroleum
    hydrocarbon contaminated soil within the Land Treatment Facility
    (LTF) at Eric Nielson Whitehorse International Airport,
    Whitehorse, B.C.  Work also includes relocation of an above
    ground fuel storage tank and paving.  Paving will require
    removal and stockpiling of approximately 75 mm of crush,
    re-grading and compaction.

    The work is to be completed by 19 October 2012.

    Value of Project: 
    To assist with bonding and insurance requirements, this project
    is estimated to cost between 
    $375,000.00 to $425,000.00.

    services provider. Firms that elect to base their bid on tender
    (solicitation) documents obtained from other sources do so at
    their own risk and will be solely responsible to inform the bid

    identification number(s) of the addenda, if any, that they have
    taken into consideration for their bid submission.
