Reinforcing and Measuring Change to Strengthen Health Services Delivery in Remote and Isolated First Nations Communities

Health Canada’s First Nations and Inuit Health Branch (FNIHB) provides primary care services directly and indirectly through transfer payments in eighty-five (85) remote and isolated communities in Northern Canada below (south of) the 60th parallel north.  
In an effort to transform health services delivery in Remote and Isolated First Nations communities by implementing recommendations in the HSDM RI report “Recognizing Strengths- Building for the Future” (April 2012) on a community-by-community basis, FNIHB is seeking a contractor to: 
1.	Propose a framework for change that supports a phased-in approach of the recommendations in the HSDM RI Report,   
2.	Implement proven change method(s) to support collaborative learning to sustain implementation related to the recommendations in the HSDM RI Report, beginning with pilot communities in at least 3 Health Canada regions and in at least 10 remote and isolated First Nations communities from October 2013 to March 2014 , 
3.	Provide learning sessions/training using/referencing proven change method(s), beginning with pilot communities in at least 3 Health Canada regions and in at least 10 remote and isolated First Nations communities from October 2013 to March 2014
4.	Create and implement detailed measurement plans to assess impact of change(s) on a community-by community basis 
5.	Design tools and processes to continually identify opportunities for improvement to sustain changes on a community-by-community basis.
