Description
The objective of the project is to conduct a mail survey of home and community care clients according to the specified methodology section, capture data from this survey, and generate data files. Please refer to sections:

2.1 A web-based version of the survey, sent directly to clients with a valid email address, and include an option for clients to be mailed a paper survey.
