Respond to and assist families and individuals with disabilities eligible for either FSCD or PDD to obtain the needed supports and services to assist with active participation within their communities through:
a) Recruitment and Hiring support and activities
b) Transition to Adulthood supports
c) Individual Planning
d) Complex Needs Coordination - training and capacity development
Contract duration
The estimated contract period will be 12 month(s), with a proposed start date of 2026/09/01.
