Description
This Request for Proposals (the “RFP”) is an invitation by the Coordinating Entity to prospective proponents to submit proposals for Community Supported Living (CSL) as further described in Section A of the RFP Particulars (Appendix D) (the “Deliverables”).   Shared Health is issuing this RFP and administering the RFP process on behalf of the Purchasers.  

Shared Health is a provincial health organization in Manitoba created to support the delivery of client-focused, consistent standards of care: “To Provide the Right Care, At the Right Time, In the Right Place”.

Shared Health and the Purchasers are seeking Service Providers to develop Community Supportive Living buildings and deliver Community Supportive Living (CSL) services through a pilot in select communities in Interlake-Eastern Regional Health Authority (IERHA), Prairie Mountain Health (PMH), and Southern Health – Santé Sud (SH-SS).  The purpose of this RFP is to select proponents who will enter into an Agreement with the Purchaser(s) to develop and deliver affordable CSL buildings and services under a proposed approach that meets the requirements of this RFP. 
The pilot of this new service will focus on individuals living in select communities in IERHA, PMH, and SH-SS.  Approximately, one-hundred and five (105) new CSL units will be added.
