Children’s Services Personalized Community Care (PCC)

Personalized Community Care (PCC) is a personalized care setting individualized to the child/youth’s complex mental health, behavioural, emotional, developmental, and trauma-impacted care needs. These care settings support a low number of children/youth within a highly structured, supervised, and staffed community care environment. Children’s Services South Region is procuring a PCC service for one youth who meets the complex criteria for this level of care. There is an urgent placement need for this youth. The procurement of this Personalized Community Care service, that is an out-of-home, individualized program, and includes a full-time, 24/7 staffed placement, located within Children’s Services, Child Intervention, South Region (South Region). This specialized care environment will be responsive to the unique and complex needs/behaviours of the identified youth. As the relationship evolves and the Youth stabilizes, there may be consideration of shared accommodations/roommate situation, should a suitable match be found. The placement should allow for opportunity to include a roommate.
