Personalized Community Care (PCC) is a community-based intensive treatment program that promotes the stabilization of mental health and behavioural crises and improved functioning in children/youth. This contract would facilitate a 24/7 living arrangement in supported environment which focuses on the individual needs of the client. This service provides children and youth with day-to-day supports/services as well as treatment interventions.
Furthermore, this service will promote stability, health, and well-being physically, emotionally and academically. Positive attachments will be built and supported. Emphasis will be placed on building healthy relationships with other people and keeping the child/youth connected to their extended family and community.
Key general deliverables:
• Safe 24/7 supported living arrangement
• Structured and normal daily routine including school or adaptive school/day-time services/supports as needed, and social, recreational and cultural activities
• Support to maintain relationships that are important to the client including connecting them to their culture, spiritual, and religious heritage and beliefs
• Foster strong support systems, community ties, family and kin relationships, educational attainment and preparation for the transition to independence and adulthood
• Support a permanency plan which includes a lifetime family that provides them safe, stable, and unconditional love and lifelong support
• Connect to formal professional services (psychological, behavioral, physical therapy, etc.) to support the effective implementation of behavioral and developmental strategies with the youth, bio-sibling and family
Services are to be delivered articulating strength-based philosophy that ties individual needs to specific interventions such as gains in measurable skills, increased productivity and pro-social behavior, improved functioning, attendance at school/academic success and a stable living arrangement in the community. General outcomes for the service will include:

• School-based functioning that supports current cognitive ability
• Increase capacity for healthy relationships
• Increase social and community connection
• Improved child development - healthy attachments, physical and intellectual development, and social and emotional health
Contract duration
The estimated contract period will be 12 month(s), with a proposed start date of 2023/04/01.
