Description
Island Health is seeking to establish contractual relationship(s) with qualified service provider(s) to support the implementation of a care support component of Complex Care Housing services. CCH seeks to better support the needs of people with overlapping mental-health challenges, substance-use issues, and trauma and acquired brain injuries and who are experiencing homelessness or are at risk of eviction. In addition to standard supports available in supportive housing such as health, personal, social and job supports, additional clinical and other services available in CCH include:
  • physical, mental-health and substance-use care;
  • psychosocial rehabilitation supports;
  • social, emotional and community supports;
  • personal care and personal living supports; and
  • Indigenous cultural supports; and, co-ordination and navigation services.
