Provision to Provide Adult Sleep Lab Equipment, Consumables & Related Service

This Request for Proposals (the “RFP”) is an invitation by the Coordinating Entity to prospective proponents to submit proposals for The Provision to Provide Adult Sleep Lab Equipment, Consumables and Related Service, as further described in Section A of the RFP Particulars (Appendix D) (the “Deliverables”).
The Coordinating Entity, as identified on the cover page, is issuing this RFP and administering the RFP process on behalf of the Purchaser(s) identified in Section B – Material Disclosures of the RFP Particulars (Appendix D).
The intent is to select, via this RFP process, ideally one successful proponent for provision of the equipment.
 Proponents are encouraged to respond with a complete product line, all or some of which may or may not form part of the final agreement(s).
 Proponents should also submit a proposal for post-warranty and/or preventative maintenance services which, at the sole discretion of the Purchasers may or may not form part of the agreement(s). If post-warranty and/or preventative maintenance do form part of the agreement, decisions on whether purchased equipment will be covered by the post-warranty and/or preventative maintenance offerings will be made on a site by site basis.
