    Description
    The Department of Health and Community Services invites proposals from interested community based service providers to provide short term residential care to individuals requiring access to timely rehabilitation and restorative care services following an acute episode or planned hospital stay prior to returning home. The goal of this service is to facilitate recovery and a return home, and to improve hospital flow. The project will be evaluated after one-year and the results of the evaluation will inform the development of one provincial program. Services will be offered up to eight weeks and are coordinated by the regional health authority (RHA) in collaboration with the client and the service provider. The RHA will help the client identify their needs, and develop a plan of care to help achieve their health goals. This community based option will reflect a philosophy of client centered care, where the dignity and autonomy of clients is respected and supported. Proponents must demonstrate that they have the required staffing, model of care philosophy, and infrastructure to provide a safe  environment for clients. As noted, this is a demonstration project which will be evaluated to inform development of a single provincial program/service option. The sites chosen will be located in area of the Province with a concentrated population, with reasonable access to a cohort of individuals meeting the criteria for the target population and with reasonable access (short travel times) to regional health authority based rehabilitation and other staff, required to support delivery of a safe model of care.  As such, the sites chosen will be located in one of the following areas: St. John’s or surrounding area, Clarenville, Gander, Grand Falls-Windsor, St. Anthony, Happy Valley-Goose Bay, Corner Brook, or Stephenville. The goal of the request for proposals process is to assess the feasibility of a new community based supportive care option to support short term rehabilitation and restorative services following an acute episode or planned hospital stay with a goal to returning to previous living arrangement.

    The Contracted Supplier will deliver a model of care that supports quality of care, quality of life and a person centered philosophy. Clients will be supported by the Contracted Supplier, in collaboration with the appropriate RHA, in appropriately accommodating each individual’s needs and any transition planning from one service to another, as required. Working collaboratively with the RHAs, clients and families, the Contracted Supplier will provide quality services that are delivered by trained staff in a safe environment. The Contracted Supplier will deliver a model of care to support individuals in a residential care setting with a goal to restore and rehabilitate function with the aim to return to previous living arrangement, specifically the client will be provided: (a) private or semi-private (maximum two clients) accessible room; (b) access to fully accessible bathroom space; (c) meals and snacks; (d) laundry services; (e) housekeeping services; (f) assistance with personal care in accordance with assessed needs; (g) medication administration; (h) access to recreational programming; (i) assistance with implementation of rehabilitation programming; and (j) supervision. The Contracted Supplier will participate in a one year demonstration project and collect data and information, as identified by the RHA and the Department of Health and Community Services. The information gathered will be used to evaluate the effectiveness of the project at the end of one year and to inform the development of a province wide program.
