    Description
    Shared Health, in collaboration with Manitoba Seniors and Long-Term Care (LTC) and the three Service Delivery Organizations (SDO), is piloting a new home and community care service, called Client-Determined Community Care (CDCC), as part of the ongoing implementation of Manitoba’s Clinical and Preventive Services Plan (CPSP). Manitoba’s CPSP has identified home and community care modernization (HCCM) as a critical priority to better support the province’s aging population and ensure individuals can access care in the appropriate settings. In parallel, the province has developed a seniors strategy through Manitoba Seniors and LTC. Collectively, these initiatives are working together to make Manitoba an ideal place to age through restorative and reablement approaches that will help individuals maximize their independence and autonomy and will ensure that Manitobans are able to stay safe in their homes and communities for as long as they choose.

    This Request for Proposals (the “RFP”) is an invitation by the Coordinating Entity to prospective proponents to submit proposals for Client-Determined Community Care as further described in Section A of the RFP Particulars (Appendix D) (the “Deliverables”). 

    Shared Health is issuing this RFP and administering the RFP process on behalf of the Purchasers.   

    The Purchasers include Shared Health in a business support role and the following Service Delivery Organizations (SDOs):

    •    Prairie Mountain Health (PMH)
    •    Interlake-Eastern Regional Health Authority (IERHA)
    •    Southern Health-Santé Sud (SH-SS)
