Reconnect Community Health Services (Reconnect) is a large multi-site non-profit community organization located in the west end of Toronto that provides an array of health services is seeking a Proponent to provide Personal Support Workers (PSW) to cover short to mid-term assignments from ½ day (4 hours) up to a couple of weeks to fill in for sick calls, vacations, etc. PSW responsibilities may include, but are not limited to the following:
• Assist clients in their units with personal care.
• Perform light homemaking and other related duties in clients’ units.
• Assist clients with personal services like online grocery shopping and put items away when delivered.
• Travel between clients an average of 3-4 times a day for a full day may be expected in the community. Provides service for multiple clients within Supportive Housing Buildings and Transitional Units.
• Report clients’ behavioral changes, physical hazards, safety, task changes or other sources of concern to Manager.
• Perform other duties as required. Attend staff meetings as required; fulfils health and safety reporting duties; undertakes training initiatives as assigned; maintains knowledge, observe and practice appropriate procedures in regards to fire, safety, quality and environmental standards;
• Provide leadership in ensuring case conferences occur as required.
• Collaborate with other appropriate service systems, organizations and individuals to ensure that services are well coordinated and meet a high standard of quality care and/or supervision;
• Identify problems, possible solutions for resource use, and collaborate for service improvement.
• Assess functional and resource needs in relation to the needs and goals of clients;
• Delivers all services in a professional manner and according to established guidelines and associated benchmarks;
• Follow all health and safety guidelines established by Reconnect;
• Fulfill other responsibilities as required.

Contract duration
The estimated contract period will be 12 month(s).
