AMENDMENT 004

This amendment is raised to make the following changes to the
Request for Proposal :

Purchase Description
Update the purchase description on the current posting.  We are
changing the colouring of the wording Canada Border Services
Agency (En/Fr), and the Canada word mark from silver to white as
per the attached updated Purchase Description dated 2014-04-15.

Pre-Award Samples
The pre-award samples made in accordance with the purchase
description dated 2013-11-14 will be accepted.  However, the
pre-production samples and the production must be made in
accordance with the updated purchase description dated
2014-04-15.

A bid already submitted may be amended prior to RFP closing time
by sending the amended correspondence to Bid Receiving Unit, the
envelope/message bearing the PWGSC file number 
and the most current closing date.

ALL OTHER TERMS AND CONDITIONS REMAIN THE SAME.

WOVEN SHOULDER INSIGNIA, LARGE & SMALL

Requirement :
The Canada Border Services Agency  (CBSA) has a requirement for
the procurement of :
Item 001 - 30007834 - Woven Shoulder Insignia, Small (English -
Drawing 1)
Item 002 - 30007835 - Woven Shoulder Insignia, Small (French -
Drawing 2)
Item 003 - 30007836 - Woven Shoulder Insignia, Large (English -
Drawing 1)	
Item 004 - 30007837 - Woven Shoulder Insignia, Large (French -
Drawing 2)	

Technical Requirements:
All units must be manufactured in accordance with Purchase
Description CBSA/ASFC-2013-08-128 dated 2013-11-14 at Annex B,
the drawings and the sealed samples.  The purchase description
shall take precedent over the sealed samples since the sealed
samples are for "guidance only".

Packaging
The shoulder insignia shall be packaged in a clear plastic
sealed bag containing 100 units per bag.  There shall be
no more than 15 packages, 1,500 units per box.  The outer
packaging shall be in accordance with good commercial standards
to ensure safe arrival at destination.

Sealed Samples :
The Sealed Samples may be viewed at the following Department of
Public Works and Government Services Regional Offices: Quebec
(Quebec); Montreal (Quebec); Mississauga (Ontario); Winnipeg
(Manitoba); Edmonton (Alberta), Vancouver (British Columbia).

Firm Quantities :
Item 001 - 30007834 - Woven Shoulder Insignia, Small (English -
Drawing 1)  	= 150,000 each
Item 002 - 30007835 - Woven Shoulder Insignia, Small (French -
Drawing 2)	=   50,000 each
Item 003 - 30007836 - Woven Shoulder Insignia, Large (English -
Drawing 1)	=   45,000 each
Item 004 - 30007837 - Woven Shoulder Insignia, Large (French -
Drawing 2)	=   15,000 each
Total firm quantities 							= 260,000 each 

Delivery to the CBSA in Ottawa, Ontario  (Total firm quantities
260,000 units)
All firm deliverables are requested to be complete in three (3)
phases :
Phase 1 - The first delivery of 65,000 units should be made by
May 31, 2014 or sooner.
Phase 2 - The second delivery of 130,000 units should be made by
June 30, 2014 or sooner
Phase 3 - The third delivery of 65,000 units should be made by
August 30, 2014 or sooner,

This requirement also includes three (3) options to purchase
additional quantities :   
Multiple amendments may be issued to exercise each option.
Option 1 - Item 005 & 006 : estimated qty 260,000 each to be
exercised within 12 months after contract award date
Option 2 - Item 007 & 008 : estimated qty 195,000 each to be
exercised within 24 months after contract award date
Option 3 - Item 009 & 010 : estimated qty 260,000 each to be
exercised within 36 months after contract award date
Enquiries :
To ensure a response before the bid closing date, all enquiries
about this requirement must be submitted in writing and received
by the Contracting Authority at least seven (7) calendar days
prior to the closing date.
