INSOLE, GYM SHOES

Requirement :
The Correctional Service Canada (CSC) has a requirement for
Insole, Gym Shoes.

Technical Requirements :
All units must be manufactured in accordance with the Purchase
Description CAG-2-0505-764A, Insole, Full length arch support
(Gym Shoes), Inmates, Correctional Service Canada provided with
the Request for Proposal and the sealed sample.

Sealed Sample :
The Sealed Sample may be viewed at the following Department of
Public Works and Government Services Regional Offices: Quebec
(Quebec); Montreal (Quebec); Mississauga (Ontario); Winnipeg
(Manitoba); Edmonton (Alberta), Vancouver (British Columbia).

Firm Quantity :
Year one - Item 001 - Insole, Gym shoes: 30,240 pairs
Year two - Item 002 - Insole, Gym shoes: 30,240 pairs (size roll
will be provided in spring 2015)
 
Estimated Quantity: "As and When Requested" :
Item 003 - Insole, Gym Shoes: 15,120 pairs 

Delivery to the Correctional Service of Canada in Laval, Quebec :
Item 001 - Year one - Delivery of the firm quantity is requested
to be delivered on April 1st, 2015.
Item 002 - Year two - Size roll to be provide in Spring of 2015.
Item 003 - "As and when requested" may be ordered on an "as and
when requested" basis for a period of 36 months
                   from Contract award date.

This requirement also includes one (1) option to purchase
additional quantity :   
Only one amendment may be issued to exercise each option.  
Option 1 - Item 004 : estimated qty 30,240 pairs to be exercised
within 36 months after contract award date

Open - All interested suppliers may submit a bid.

Enquiries :
To ensure a response before the bid closing date, all enquiries
about this requirement must be submitted in writing and received
by the Contracting Authority at least seven (7) calendar days
prior to the closing date.
