Requisition Number - EQ734-131394/A Project Number - R.048689.002 SCOPE OF WORK : The Work of this Project comprises the demolition of an existing 170 sq.m. one storey building, removal of an adjacent loading dock, and the construction of a new kitchen facility building of approximately 822 sq.m. attached to the existing MHU building. The new building includes a mezzanine level. An exit path through the construction site and institutional fire route must be maintained throughout construction activities at Millhaven Institution, Bath Ontario. Further identified as PWGSC Project Number R.048689.002. Work does not include the supply of kitchen equipment. The Contractor shall perform and complete the Work within 41 weeks from the date of notification of acceptance of the offer. The site visit for this project is MANDATORY. Arrangements have been made for a tour of the work site. The site visit will be held on the 17th October 2012 at 10:00 AM. Bidders are to meet at "Sallyport" CSC Millhaven Institution, Bath ON. Bidders are requested to communicate with the Contracting Authority, before 12:00 PM 16th October, 2012 to confirm attendance and provide the name(s) and number of people who will attend. The representative of the bidder will be requested to sign Site Visit Attendance Sheet at the visit. Bidders who do not attend or send a representative will not be given an alternative appointment. Any clarifications or changes to the bid solicitation resulting from the site visit will be included as an amendment to the bid solicitation. Contractors must bring picture ID. No pictures will be allowed and no cell phones can be brought into the Institution. The site visit for this project is MANDATORY. The representative of the bidder will be required to sign the Site Visit Attendance Sheet at the site visit. Bids submitted by Bidders who have not signed the attendance sheet will not be accepted. ***Please Note: This site visit will serve as an opportunity for contractors to walk around the site and view the area(s) where the work is to be completed. Enquiries of a technical or contractual nature will not be answered during the site visit. Drawings and Specifications are now available for viewing and downloading in a Portable Document Format (PDF) or in a standard paper version. Standard Terms and Conditions for this procurement are incorporated by reference into, and form part of the Tender and Contract Documents. If such a list has not been received by the time the evaluation of bids is completed, Canada will inform the Bidder of a time frame within which to provide the information. Failure to provide such a list within the required time frame will render the bid non-responsive. Bidders must always submit the list of directors before contract award. Canada may, at any time, request that a Bidder provide properly completed and Signed Consent Forms (Consent to a Criminal Record Verification form - PWGSC-TPSGC 229) for any or all individuals named in the aforementioned list within a specified delay. Failure to provide such Consent Forms within the delay will result in the bid being declared non-responsive. A COMPLETE LIST OF EACH INDIVIDUAL WHO IS CURRENTLY ON THE BIDDER'S BOARD OF DIRECTORS.
