    Bidders must submit the following duly completed certifications
    as part as their bid.
    
    CODE OF CONDUCT CERTIFICATIONS - CONSENT TO A CRIMINAL RECORD
    VERIFICATION
    
    Bidders must submit with their bid:
    a complete list of names of all individuals who are currently
    directors of the Bidder (as Annex C); and
    
    a properly completed and signed form Consent to a Criminal
    Record Verification (PWGSC-TPSGC 229 -
    http://www.tpsgc-pwgsc.gc.ca/app-acq/forms/229-eng.html), for
    each individual named in the list (see Sample Form at Annex D).
    
    The above should be completed and submitted with the bid by the
    bid solicitation closing date.  If the above is not completed
    and submitted with the bid by the bid solicitation closing date,
    the Contracting Authority will so inform the Bidder and provide
    the Bidder with a time frame within which to meet the
    requirement. Failure to comply with the request of the
    Contracting Authority and meet the requirement within that time
    period will render the bid non-responsive.
    
    Work Description:
    Work of this contract comprises the provision of Window Cleaning
    services including all labour, material, equipment, supervision
    and transportation for Canada Border Services Agency, located at
    Huntingdon Border Crossing, 0 Sumas Way, Huntingdon, B.C. and
    Pacific Border Crossing, 28 - 176th Street, Surrey, B.C.,
    
    Mandatory Site Visit:
    It is MANDATORY that the bidder attend the site visit at the
    designated date and time to examine the scope of the work
    required and the existing conditions. A maximum of two (2)
    representatives per bidder will be permitted to examine the
    sites.
    
    The site visit will be held on August 17, 2012 at 9:00a.m.
    Bidders are to meet at Pacific Border Crossing Commercial Lobby
    located at #28 - 176th Street Surrey, BC.  Bidders can park in
    either of the two Public parking lots on site. After the tour of
    Pacific Border Crossing,  bidders will go to Huntingdon Border
    Crossing to tour that facility.  Bidders who, for any reason,
    cannot attend at the specified date and time will not be given
    an alternative appointment to view the site and their proposals,
    therefore, will be considered as non-responsive. NO EXCEPTIONS
    WILL BE MADE.
    
    As proof of attendance, at the site visit, the Technical
    Authority will have an Attendance Form which MUST be signed by
    the bidder's representative. It is the responsibility of all
    bidders to ensure they have signed the Mandatory Site Visit
    Attendance Form prior to leaving the site. Proposals submitted
    by bidders who have not attended the site visit or failed to
    sign the Attendance Form will be deemed non-responsive.
    
    Mandatory Contractor's Certification:
    Contractor's window cleaners must be safety trained and
    certified by the Exterior Building Contractor's Association or
    other acceptable organization that is recognized by WCB prior to
    working on the sites.
    
    SECURITY REQUIREMENT FOR CANADIAN SUPPLIER:
    Mandatory Security Clearance:
    The Contractor must, at all times during the performance of the
    Contract, hold a valid Designated Organization Screening (DOS),
    issued by the Canadian Industrial Security Directorate (CISD),
    Public Works and Government Services Canada (PWGSC).
    
    The Contractor personnel requiring access to sensitive work
    site(s) must EACH hold a valid RELIABILITY STATUS, granted or
    approved by CISD/PWGSC. Until the security screening of the
    Contractor personnel required by this Contract has been
    completed satisfactorily by the CISD, PWGSC, the Contractor
    personnel MAY NOT ENTER sites without an escort.
    
    Subcontracts which contain security requirements are NOT to be
    awarded without the prior     written permission of CISD/PWGSC.
    
    The Contractor must comply with the provisions of the:
    (a) Security Requirements Check List and security guide (if
    applicable), attached at Annex B
    (b) Industrial Security Manual (Latest Edition).
    
    Period of Contract:
    The period of any resulting Contract shall be for a period of
    two (2) years with three (3) options to extend the contract each
    for an additional consecutive twelve (12) month period.
    
    Option to Extend Contract:
    The Contractor hereby grants to Canada three (3) irrevocable
    options to extend the term of the Contract each for an
    ADDITIONAL CONSECUTIVE TWELVE (12) MONTH PERIOD, under the same
    terms and conditions as contained in the Contract. It is to be
    noted that Canada is not obliged to exercise any of these three
    (3). The exercise of any option will be at Canada's sole
    discretion, by providing notification in writing through a
    formal Contract Amendment to the contractor prior to the
    contract expiry date.

    their own risk and will be solely responsible to inform the
    tender calling authority of their intention to bid. In all
    cases, bidders must indicate (when requested) in the space
    provided on the tender form the identification number(s) of the
    addenda, if any, that they have taken into consideration for
    their bid.
