Description
On behalf of the Employee Benefit Plans Board of Trustees, 3sHealth is initiating this Request for Proposal (RFP) to select a partner to provide rehabilitation services. Our goal is to support plan members who are on an approved long term disability (LTD) claim to restore function and return to work.
3sHealth intends to award a formal written agreement for three (3) initial years, with the option to renew two (2) additional year terms, upon mutual agreement. 3sHealth reserves the right to award to a single vendor or split the award amongst multiple vendors that can meet the specifications and/or service requirements as stated herein.
