    This requirement is for the department of Fisheries and Oceans Canada.

    Title: Health Care services at the Canadian Coast Guard College, Sydney NS

    Introduction:

    To provide a bilingual Health Care Specialist for the Officer Cadets on site at the Canadian Coast Guard College in Sydney, NS.

    Period of Contract:

    The period of contract is from date of award to May 31, 2018. The services will be delivered on site for 12 hours per week, 48 weeks per year.

    Option to extend the contract:

    The Contractor grants to Canada the irrevocable option to extend the term of the Contract by up to one (1) additional one (1) year period under the same conditions. The Contractor agrees that, during the extended period of the Contract, it will be paid in accordance with the applicable provisions as set out in the Basis of Payment.

    Canada may exercise this option at any time by sending a written notice to the Contractor at least 15 calendar days before the Contract expiry date. The option may only be exercised by the Contracting Authority, and will be evidenced for administrative purposes only, through a contract amendment.

    Security Requirement:

    The bidder company and the proposed resource(s) must meet the following security requirements at the time of Proposal submission:

    1) A valid Designated Organization Screening (DOS) issued by the Canadian Industrial Security Directorate (CISD), Public Works and Government Services Canada.

    2)Reliability Status for the resource(s) (issued by the Canadian and Industrial Security Directorate at PWGSC).

    MANDATORY REQUIREMENTS

    Mandatory Criteria

    M1

    Health Care Specialist that is a Physician. Must meet the requirements for a Full Licence http://www.cpsns.ns.ca/Registration-Renewal/Licence-Type/Full-Licence as set out by the College of Physicians & Surgeons of Nova Scotia. Provide copy of licence.

    Must be a certified Transport Canada Marine Medical Examiner https://wwwapps.tc.gc.ca/saf-sec-sur/4/mme-mdm/eng/marine-medical-examiners/search

    M3

    Must provide proof of Commercial General Liability Insurance.

    M4

    Must provide proof of Medical Malpractice Liability Insurance.

    M5

    Must demonstrate collaboration with an off-site family physician to provide a range of primary care services.
