Burling Lift Bridge. Deck Repairs, Welding and Maintenance Services. Project No. R.090211.001 Requirement Provide all necessary labour, tools, equipment, materials, safety devices, and supervision required to provide regular maintenance services such that the Burlington Lift Bridge will be safe, reliable, and meet operational requirements SCOPE OF WORK Work of this Contract comprises Steel Deck Grating Repairs. General items of work include but are not limited to: Re-welding of existing cracked button welds. Restoring broken connections between steel grating members. Grating Panel Replacement. The timing of this work will be at the full discretion of the Departmental Representative. Welding Repair Repairs for the steel deck grating welding failures are to continue to keep the deck safe to traffic until a solution for total deck replacement is implemented. Ensure the repairs are compatible with the existing deck grating with respect to material, roughness, and traction. The contractor shall cooperate with consultant representative for confirming the appropriate repair type (i.e. Repair type: A, B, C, D, and E). The construction work for the repairs shall be carried out to allow the facility to remain fully operational at all times and the Bridge operation shall not be impacted. This includes minimizing disruption to Bridge lifting for navigation purposes, vehicular traffic, and pedestrians. Panel Replacement Deck panels experiencing extensive and repeated weld breaks may be required to be replaced in kind. The construction work for the replacement of panels shall be carried out to allow the facility to remain fully operational at all times and the Bridge operation shall not be impacted. This includes minimizing disruption to Bridge lifting for navigation purposes, vehicular traffic, and pedestrians. Comply with Health & Safety, Environmental Regulations, and Security Requirements. Traffic Control Definition: Provide traffic control to close one lane of traffic. Maintain one lane of traffic in each direction at all times. Description: DR may request traffic control in conjunction with other planned work. Scope Crew Size: Provide sufficient labour to install, maintain, operate and remove traffic control in accordance with MTO Traffic Manual Book 7. Materials and Equipment Signs and traffic control devices to MTO Traffic Manual Book 7 Procedure Install, maintain, operate and remove traffic control in accordance with Section 7 of this Statement of work. Close one lane of traffic. One traffic lane in each direction to remain open at all times. Provide traffic control and management plan to DR minimum 2 days prior to start of work. Scheduling: Schedule and coordinate work with DR as required. The Contractor must perform and complete the Work by March 31, 2020. OPTIONAL SITE VISIT There will be a site visit on January 25, 2019 at 10:00 A.M. Interested bidders are to meet at the site, Burlington Lift Bridge, Control Building. 1157 Beach Blvd, Hamilton, Ontario L8H 6Z9. The representative of the Bidder should sign the Site Visit Attendance Sheet at the site visit. Safety Attire: In order to be guaranteed access to the site visit all persons should have the proper personal protection equipment (safety glasses, footwear, vests and hard hats etc.). Contractor`s personnel/individuals who do not have the proper safety attire may be denied access to the site. Bidders are requested to register 24 hours prior to the site visit, with the Contracting Officer, Sheila Dhanna at sheila.dhanna@tpsgc-pwgsc.gc.ca to confirm their attendance and provide the name(s) of the person(s) who will attend. IMPORTANT NOTICE TO BIDDERS IMPORTANT CHANGES TO GI01 "CODE OF CONDUCT AND CERTIFICATION OF - BID" R2710T, GENERAL INSTRUCTIONS TO BIDDERS (REFER TO SPECIAL INSTRUCTIONS TO BIDDERS SI02) Bidders should provide, with their bid or promptly thereafter, a complete list of names of all individuals who are currently directors of the Bidder. If such a list has not been received by the time the evaluation of bids is completed, Canada will inform the Bidder of a time frame within which to provide the information. Failure to provide such a list within the required time frame will render the bid non-responsive. Bidders must always submit the list of directors before contract award. Canada may, at any time, request that a Bidder provide properly completed and Signed Consent Forms (Consent to a Criminal Record Verification form - PWGSC-TPSGC 229) for any or all individuals named in the aforementioned list within a specified delay. Failure to provide such Consent Forms within the delay will result in the bid being declared non-responsive. A COMPLETE LIST OF EACH INDIVIDUAL WHO IS CURRENTLY ON THE BIDDER'S BOARD OF DIRECTORS.
